Health Care Proxy

What is a Health Care Proxy?

A Health Care Proxy is a simple but vital form in estate planning.  This form allows you to name someone you trust to make health care decisions for you according to your wishes, if, for any reason and at any time, you become unable to make or communicate those decisions yourself.  If, because of an accident or illness you cannot make your own health care decisions you have empowered someone you trust to put into action the necessary treatment.  A properly executed health care proxy, along with a durable power of attorney, can avoid the need to come to Probate Court for a guardianship.

Health Care Proxy Form and Instructions

Through the courtesy of Massachusetts Health Care Decisions, a Proxy form has been provided on-line to promote and assist with the completing this document.  This form is provided for personal use.
